Candidates shall be persons who have exhibited a devotion to the advancement of health, physical education, recreation, dance, and/or sport as exemplified by their leadership and industry.

The Midwest Honor Award recipient is encouraged to complete an AAHPERD Honor Award and/or Luther Halsey Gulick Award application (including letters of recommendation from AAHPERD members and a publicity form) by December 1.
The Honor Award or Gulick Award materials should be sent to the Recognition Awards Committee, American Alliance for Health, Physical Education, Recreation, and Dance, 1900 Association Drive, Reston, VA 20191, for national consideration.
