Chapter Registration Forms and Membership Fees
Each year, each dance honor society chapter must submit the registration forms and membership dues to continue chapter activity.

New Chapter
If you wish to establish a new chapter of the Nu Delta Alpha dance honor society, please complete the Letter of Commitment. Then submit the Chapter Registration and Membership Forms found below.
Renewing Chapter
To renew your chapter for the coming school year/ semester, fill out the Chapter Registration and Membership Forms below. This must be completed each school year/ semester the chapter wishes to receive certificates and awards for induction.

Membership Fees
Chapter Fee- This is a non-refundable, one-time fee that the chapter pays upon initiation into the dance honor society.
$200.00 University/ College chapter
$100.00 High School/ Middle School/ Studio chapter
Student Initiation Fee- First Year- For new members to the dance honor society. This fees includes, upon the completion of 30 points: dance honor society certificate and dance honor society pin.
$22.00 per member
Students Fee for Second and Third Years- For members who have been a part of the dance honor society for more than one year and are not graduating seniors. This fee includes: dance honor society certificate.
$15.00 per member
Student Fee for Graduation Year- For graduating senior members of the dance honor society. This includes new members that are also graduating seniors. This fee includes: dance honor society certificate.

Optional Items
Individual Plaque- For graduating College/ University seniors only. Members will receive individual plaque instead of certificate.
$55.00 per plaque
Framed Dance Honor Society Certificate- For graduating High School seniors only.
$30.00 per framed certificate
Honor Cords for College/ University graduation: Maroon/ Gold double intertwined and double twisted- For graduating College/ University seniors. Will receive in addition to dance honor society certificate.
$20.00 per honor cord
Honor Cords for High School graduation: Maroon/ Silver single twisted- For graduating High School seniors. Will receive in addition to dance honor society certificate.
$10.00 per honor cord
Honor Stoles for graduation: Silver/ Maroon (High School), Gold/ Maroon (College/ University)- For graduating senior members. Will receive in addition to dance honor society certificate.
